1) Purchase the specially marked Physicians Formula Physician's Formula 100% Natural Origin Jumbo Mascara product between 12/1/10 and 12/31/11.

2) Print out and mail this completed form along with the TRY ME FREE label attached to the product and your original sales receipt with purchase price circled.

3) Rebate must be postmarked by 1/14/12.
