1) Purchase the specially marked Physicians Formula Mineral Wear Correcting Primer product between 12/1/10 and 11/30/11.

2) Print out and mail the completed rebate form along with the TRY ME FREE label attached to the product and your original sales receipt with purchase price circled.

3) Offer expires November 30th, 2011.Rebate must be postmarked by 12/15/11.
